Salt Lake County Vital Records provides certified copies of birth and death certificates, as well as abstracts of marriage and divorce records, to individuals eligible to receive them.
Vital records offices are located in three public health clinics in Salt Lake City, West Jordan, and West Valley City. You may also request a record online.
Current (nonhistoric) vital records are protected, private information and are only available to the person listed on the record and the person's:
- current/surviving spouse
Other individuals are required to show legal proof of need.
Historic vital records become public at varying times, depending on the type of record.
Before you request a vital record, be sure the record you're looking for is available at our offices and that you’re eligible to receive it.
Utah does not offer a free birth certificate for newborns. Many hospitals offer a souvenir certificate, but this copy is not legally valid and is not provided by our office. Vital Records Offices provide certified copies to be used as a legal document throughout a person’s lifetime.
