Home & Community Based Care Provider Information
New Request for Applications (RFA) 2023!
Aging & Adult Services (AAS) invites applications for its new provider contracts. Through these contracts, Case Managers will coordinate services and supplies for clients enrolled in one of four home-based programs: The Alternatives Program, Caregiver Support Program, Veterans Directed Care, and Refugee Program for Older Adults. These programs support adults and caregivers who wish to live in an independent setting and can continue to do so with case management, supplies, and services.
Applications are received on an ongoing basis starting August 14, 2023. Providers can deliver services and purchase goods through one or more of the following contracts:
- Adult Day Care
- Assisted Living Facility Respite
- Emergency Response Systems
- Environmental Adaptation
- Fiscal Intermediary
- Home Health Care
- Medication Reminder Systems
- Nursing Home Respite
- Personal Care
- Specialized Medical Equipment, Supplies and Assistive Technology Items
A Pre-application Orientation will be held on Monday, August 21st, 2023 at the Salt Lake County Government Center on 2001 South State Street, Salt Lake City, Utah 84190 in Room AG S2-830. Orientation starts at 9:00 am and ends at 12:00 noon.
